The Newfoundland and Labrador Centre for Health Information (NLCHI) maintains this Digital Travel Documentation application on behalf of Public Health and the Government of Newfoundland and Labrador for the purpose of collect Self-Declaration Forms and confirming traveller entrance eligibility is within current COVID- 19 related measures defined by the Chief Medical Officer of Health (CMOH). NLCHI is committed to protecting the privacy of individuals completing these forms in compliance with the Access to Information and Protection of Privacy (ATIPP) Act.

Personal information means recorded information about you as an identifiable individual but is not limited to contact information (employee number, email address, phone number, name, age, and gender).
Collection, Use & Disclosure of Information
When completing this form, you will be asked to provide certain personal information, including your name, date of birth, travel information, and additional information.
The information you provide may be used and disclosed for public health surveillance, health system planning, resource allocation, program evaluation and delivery, and monitoring public health and safety as permitted by sections 61, 66 and 68 of the ATIPP Act. Other uses and disclosures as required by law may also occur. Where possible, identifying information will be removed prior to any use or disclosure.

If your complaint is not resolved to your satisfaction, the ATIPP Act provides a mechanism for independent review by contacting the Office of the Information and Privacy Commissioner – https://www.oipc.nl.ca/
